For a limited time, we are offering a $100 rebate via check. In order to receive this rebate, you have to meet the following criteria. All requirements have to be met in order to qualify:
1) Digital Video (not analog) on the small digital video tapes with at least 10 minutes, preferably more, of the customer using the device with or without audio. (Or raw AVI format on CD or DVD data disc)
2) 10 (digital only – no conventional photo printouts) pictures of the installation location and the consumer using the product. Pictures have to be taken from a 5 Megapixel or higher camera.
3) Written testimony of one paragraph or more on how the unit has helped you or your family member.
4) Signed waiver (Word Document) stating we can use the video, testimonies and pictures on the website or other media.
